Birth Certificate Application

Ensuring a streamlined process for your child’s birth certificate issuance is paramount to us. Please closely follow the guidelines below:

1- Child Name Consent and Details Form

Complete the hospital-provided form with your child's name and relevant details.

2- Parent Details

For the birth certificate, the parents' names and addresses will be as they appear on the Aadhar card.

3- Document Submission

Submit clear photocopies of both parents' Aadhar cards. Should discrepancies exist or if updates are required on your Aadhar card, ensure they are corrected before submission.

4- Acknowledgment and Next Steps

After submitting the child name consent form, you will receive an SMS containing an acknowledgment number. Use this number at your nearest Mee-Seva centre to continue the birth certificate application process.

5- Timeline

The SMS acknowledgment will arrive within 15 days of submitting the child's name consent form. In case of technical or server issues it might take longer.

6- Review and Consent

Ensure all details and spellings on the hospital form are accurate before submission. By completing and submitting the form, you consent and acknowledge that: - Any errors or name changes after submission won't be rectified by the hospital.

- All costs of corrections, if any, will be borne by the parents.

- The entire process, including any steps at a government E-Seva or Mee-Seva centre, is to be done by the parents only

7- Document Submission Via Email

Alternatively, Aadhar card IDs can be emailed to "drarshadshospital@outlook.com"

Previous Deliveries & Late Submission

For families who delivered at our hospital but didn’t submit documents within 11 months from the date of delivery or birth, the following applies:

– According to GHMC regulations, parents must process birth certificates at a government E-Seva centre.

– Parents are responsible for all late submission fees and processes associated with the delay.

– The hospital will only provide the necessary birth form as mandated by the law.

Accuracy in your submission is essential to avoid future challenges. The hospital won’t be held responsible for any discrepancies or changes in names and spellings. Verify all details thoroughly before submitting.
